An initiative of the Institute on Disability at the University of New Hampshire designed to identify barriers to community living and to develop improved ways of providing community supports and services to enable older adults and individuals with disabilities to live and participate fully in their communities.
Real Choice Systems Transformation is a grant funded by the Centers for Medicare and Medicaid Services and administered by the New Hampshire Department of Health and Human Services under contract through the Institute on Disability at the University of New Hampshire.
